The MHRA approved the higher-dose regimen in January 2026 and the new single-dose 7.2mg pen on 14 April 2026; since June 2026 it comes as four single-use pens per month (one per weekly injection, disposed of after each use). In the 72-week STEP UP phase 3b trial (1,407 adults with obesity, across 11 countries), people taking semaglutide 7.2mg alongside diet and exercise lost an average of 20.7% of their body weight, compared with 17.5% on the standard 2.4mg dose. Notably, about one in three participants (33.2%) lost 25% or more — results in a range previously associated only with bariatric surgery or the highest dose of Mounjaro.

The high dose is for adults with obesity (BMI 30 or above) who need more support than the standard 2.4mg maintenance dose — typically people whose weight loss has plateaued despite good adherence. It isn't a starting dose: you reach 7.2mg only after titrating up through the standard schedule and at least a month at 2.4mg, with each step confirmed at your face-to-face review. Side effects are the same gastrointestinal family as other doses and remain mostly mild to moderate, but they are more likely at higher doses — which is exactly why our in-person monthly monitoring matters.
